CRBA Procedures and Required Documents

When applying for a CRBA, please ensure you have all the documentation as listed below.  Complete, but do not sign the application forms as they must be signed in front of a consular officer during the interview.  The DS-11 Application for a U.S. Passport is only required if you are also applying for a U.S. passport for your child at the same time as the CRBA application.

  • Completed CRBA application form (DS-2029) – filled out in eCRBA
  • Completed passport application form (DS-11) (if applicable)
  • Child’s birth certificate (volet 3)
  • Father’s U.S. passport (or local passport or ID if not a U.S. citizen)
  • Mother’s U.S. passport (or local passport or ID if not a U.S. citizen)
  • Naturalization certificate (if applicable)
  • Marriage certificate (if applicable)
  • Divorce certificate of any previous marriage(s) (if applicable)
  • Proof of physical presence in the United States of the U.S. citizen parent(s)
  • Consent Form DS-3053 (if one of the parents will not be present at the interview)

Please review our Online Guide for Completing the DS-2029 CRBA Application and the Online Passport Application Instructions to ensure you are completing the forms correctly. Both the DS-2029 and DS-11 forms must be completed online; handwritten forms will not be accepted.

Please note, for U.S. citizen parents who will not be present at the interview, the Forms DS-2029 and DS-3053 must be signed and notarized in the United States or at a U.S. Embassy or Consulate.  The applying parent must bring the original, notarized, and signed document to the interview.

Attention: The DS-2029 application must be fully completed, including the section on precise periods of time in the United States before the child’s birth. If the instructions above are not followed or documents are missing, your appointment request will be rejected.

At the Interview: You must bring with you to the interview the originals of all documents and two photocopies of each of the following documents:

  • Father’s passport or ID
  • Mother’s passport or ID
  • S. naturalization certificate
  • Child’s birth certificate
  • Marriage certificate (if applicable)

Please also bring supporting documents for proof of your presence in the United States.  This may include, but is not limited to, previous passports with entry/exit stamps, round-trip plane tickets, leases, and school transcripts.

Fees: The CRBA application fee is $100, and the first passport application fee is $135.  The CRBA fee may be paid online during the eCRBA process.  The passport fee is paid at the time of the interview.  Both fees are non-refundable. Both cash (USD and FCFA) and credit card are accepted.

Paper CRBA:  If you do not qualify for eCRBA, please send one email with all of the documents attached in PDF format to CRBABamako@state.gov.  Paper CRBA applications are scheduled solely by email to CRBABamako@state.gov – do not call or make an appointment online.

We are unable to schedule appointments without a complete application package. If the instructions above are not followed or documents are missing, your appointment request will be rejected.
