Report a Lost or Stolen Passport

Please report any lost or stolen passport to the Consular Section as quickly as possible.  No appointment is necessary.

  • During working hours: Please call the Consular Section at (+223) 20-70-25-05.
  • After hours: You may contact the Consular Section the next business day or come in for emergency assistance.

Keep in Mind

Prompt reporting can help prevent identity theft.  Once a passport is reported as lost or stolen, you will need to apply for a replacement passport and foreign visas.

Passports reported lost or stolen are invalidated and can no longer be used for travel. The information you provide will be entered into the Consular Lost/Stolen Passport System.

Once a passport is reported lost or stolen, it cannot be re-validated.  If you recover the passport after you have reported it lost or stolen, please submit it to the Consular Section, where it will be cancelled and returned to you.

In cases of extreme emergencies or when a citizen is in transit, a limited validity emergency passports can usually be issued the same day.  Otherwise, a full validity passport can usually be received within 10 days.

The Consular Section is unable to assist you in obtaining foreign visas.  Any valid visas that are needed for travel that were in the lost or stolen passport are the responsibility of the bearer.


Replacing an Adult Passport

After reporting a lost or stolen adult passport, you can replace the passport at the U.S. Embassy Bamako. To do so, you must appear in person at the Embassy and bring the following items:

Please complete form DS-64, Statement Regarding a Lost or Stolen Passport. Fill in as much of the form as possible and answer all the questions in detail.

Please complete form DS-11. DO NOT SIGN it until instructed to by a consular officer.

Please provide a police report showing your previous passport as being one of the items lost or stolen.

One full-face 2 inch by 2 inch color photo taken within the last six months. The photo must meet these specifications.

Please provide proof of U.S. citizenship (original AND one copy), if possible, such as birth certificate from the U.S., a Consular Report of Birth Abroad, or a Naturalization Certificate.

Please bring funds to pay for the Application Fee. The payment can be made at the U.S. Embassy in cash or credit card. U.S. Dollars or CFA are accepted.

You must have the following documents ready before making an appointment.  Failure to bring the required documents may result in significant processing delays.

Replacing a Minor Passport (under age 16)

After reporting a lost or stolen adult passport, you can replace the passport at the U.S. Embassy Bamako.  To do so, the minor must appear in person at the Embassy and follow the steps below:

  • Evidence of Parental Relationship
  • Photo Identification of Parent/Guardian
  • Parental Consent
  • Progression Photos – if needed
